Our team members will work closely with your healthcare providers to develop a program specific to your needs.   As a member of the National Home Health Quality Campaign LifeCare is able to implement the latest proven methods and best practices to treat and prevent chronic conditions developed by the top quality improvement organizations.

  Our programs include but are not limited to specialized management of Heart disease, pulmonary conditions,  Diabetes, ALS, MS, Alzheimer’s & Dementia.  Our programs involve preventative measures and treatment plans for both our medical and non-medical team under the coordination of the Registered Nurse.

LifeCare Nurses will monitor signs and symptoms, manage medication regimen, develop a healthy nutritional plan, define tasks for aides and caregivers to follow and report on.

LifeCare Therapists will evaluate home settings and will make recommendations on home safety improvement and adaptation techniques will develop short and long-term goals for improving physical activity, will develop appropriate exercise programs and will teach Aides / Caregivers with proper techniques for ongoing home exercise programs. 

LifeCare Aides and Caregivers will follow the aides care plan to assist with activities of daily living while focusing on disease-specific tasks of monitoring signs and symptoms, recording vitals, ensuring proper positioning, encouraging exercises, preparing appropriate nutritious meals, documenting and reporting on changes in the conditions to the Nurse Care Managers.

LifeCare Disease Management Programs

LifeCare Heart

LifeCare Heart program was developed to address care needs for our patients with Chronic Heart Failure (CHF) and heart disease. 

LifeCare Lungs

LifeCare Lungs program was developed to help patients with Chronic Obstructive Pulmonary Disease (COPD) to improve quality of their daily life, to teach and assist with skills to manage chronic disease in the home environment.

LifeCare Diabetes

LifeCare Diabetes program was developed to address care needs for people with type I and II Diabetes. LifeCare team is trained to provide Ongoing patient self-management education and support are critical to preventing acute complications and reducing the risk of long-term complications.

LifeCare Joints

LifeCare Joints Program was developed for patients who undergo knee and hip joint replacement. The program is focused on improving mobility, strength, and balance so that patients can get around and continue with outpatient therapy if needed.

LifeCare Memory Care 

LifeCare Memory Care programs were established to provide care at home for patients with Dementia and Alzheimer.  LifeCare is using evidence-based practice recommendations developed by the Alzheimer's Association to provide specialized care in the home or facility setting.

ALS and MS Support Programs

LifeCare also offers staff training to provide support and management of patients with neurodegenerative diseases, such as ALS and MS.
