Care Coordination

We are focused on care coordination to ensure the quality and safety of the care delivered. Our Care Coordinators serve as a central point to ensure that your needs are known and communicated at the right time and to the right people. They focus on communicating and sharing information with all parties involved in your care and health: your family members, healthcare providers, community organizations, etc. The care plan is updated based on evolving needs and monitored for changes. Changes in status are communicated to all involved parties. At the time of hospitalization or transfer from a facility, our Care Coordinators ensure that healthcare providers have up-to-date patient information, and are able to discuss the intervention and treatment plans with healthcare providers.

Client Advocacy

LifeCare works with a trusted network of certified and experienced Geriatric Care Managers and Patient Care Advocates, who are trained social workers or registered nurses.

These professionals can:

• protect your rights to an accurate diagnosis,
• collaborate on your behalf with healthcare providers,
• help you navigate acute illnesses,
• and reduce the occurrence of medical errors.

Geriatric Care Managers and Patient Care Advocates work with you and family members and are able to assist with management of:

• Medicare and private insurance paperwork,
• planning for the future and use of assets,
• helping with a move to a new home,
• scheduling and supervising doctor visits,
• offering support and counseling
